The degree of torsion the testicle not managed and treated may play a role in the viability of the testicle over time. Testicular viability decreases significantly after 6 hours from onset of symptoms so diagnostic testing should not delay treatment or Surgical Intervention by your doctor and we called this the Role Of "6 Golden Hours"


What are the Types of Torsion?

There are 2 Types of Testicular Torsion

1- Intravaginal torsion :
occurs due to Bell Clapper Deformity and due to inappropriate high attachment of the tunica vaginalis to the testicle.
Most Common in adolescents.

2- Extravaginal torsion: 
Most Common in neonates.
The tunica vaginalis is not attached to the gubernaculum so the spermatic cord and the tunica vaginalis undergo torsion as one unit.

 

Some Anatomical Points You Should Know:

The tunica vaginalis attachment: is normally to the posterolateral aspect of the testicle, at which the spermatic cord is not very mobile.

Bell clapper deformity: it is the deformity at which the attachment of the tunica vaginalis to the testicle is abnormally high so the spermatic cord can rotate within it, which may lead to intravaginal torsion.

 

How I Suspect That I have Torsion?

If you Have the following Symptoms you may have Testicular Torsion and Should harry up to the hospital before your testis die and lose it and lose your fertility forever :

1- Acute Scrotal Pain and Tenderness.

2-Scrotal Oedema.

3- Scrotal Erythema, redness, hotness.

4- your testis is very hard and not as usual.

5- if you have abdominal pain, Nausea, and Vomiting.

6- if you have a Groin Pain.

7- if your testis Lie Horizontally and not Vertically as normal.

 

Remember the role of 6 Golden Hours and don't take analgesics and wait in home you may lose your testicle if not managed and treated early.


treatment Modalities

Treatment: by Urgent Exploration by the Urologist, untie the Twist to relive the occlusion of the blood supply and let the testicle to survive and to fix the testicle in the scrotum by an operation called Orchiopexy to prevent Future twist.

If you have torsion and be, late the operation may not help you if the testicle is dead and we proceed to testicle removal operation called Orchidectomy.
